Once needs are identified RIDCA assists in the development and co-ordination of services including links to services such as supported accommodation, supported living, vocational support, planned respite and Community Liaison Teams. For RIDCA clients this may mean a change in service provider to a RIDSAS. If the person meets the eligibility criteria for RIDCA or are made a care recipient under the IDCCR Act they are able to access services from the RIDSAS (Regional Intellectual Disability Supported Accommodation Service). It is the function of the RIDSAS to provide a community based residential service with intensive supports ensuring security and physical safety within the person’s home and the community. Care recipients under the IDCCR Act will have their care requirements determined by the court through the signing off of Care and Rehabilitation Plans and Care Programmes. Changes to these programmes will need to be taken back to Court for approval. An individual plan includes; the person’s short and long term goals and timeframes for achievement, the supports, activities and inputs required to achieve the goals, family/whanau/friends/advocate involvement, skills attainment, recognition of Maori and other cultural aspects, assignment of responsibilities for implementing goals, acknowledgement of risk factors in the achievement of goals, transition goal setting. |
