Needs Assessment

A needs assessment is a process that helps to identify your strengths, abilities, needs and goals.

Who is involved in the assessment?
An Assessment Facilitator will meet with you in the place of your choice. Family/whanau, friends, caregivers, or an independent advocate can be involved for support if you choose.

Access Ability has Maori and Pacific Assessment Facilitators who are available to work with Maori and Pacific people with disability related needs.

What happens in the Needs Assessment process?
The Assessment Facilitator will discuss your situation with you (including your family/whanau if appropriate). A report will then be put together that identifies your needs.

These needs might be:
What supports do I need right now:
  • to look after myself, my family and my home
  • to sleep safely at night
  • to be in the community
  • to be socially active
  • to keep in touch with my friends and family
Do I need:
  • a place to live
  • a break from my family
  • something to do during the day
While a Needs Assessment does not guarantee the provision of all the services you would like, it is a useful process for identifying what needs are most important to you.
